flat on his back是副词短语也可以看做形容词短语不是动词短语这里是作方式状语的修饰 spend all his time 也可以看做形容词短语作伴随状语
平躺着度过他所有的时间

平躺在床上度日。flat应该是副词，修饰动词spend的，根据词典查了意思是（尤指贴着另一表面）平直地。on是跟在后面his back的,on his back.
