A从来不用,B一般指敲门,C指在门上,比如There is a picture on the door,意为门上挂着一幅画.

in是在...里面，不可能有某个人在门里吧
at the door一般指敲门，有时也可与knock连用，如：There's someone knocking at the door.
on是在...上面，不可能有一个人在门上吧，这不符合常理，因此选B是最恰当的。
