目的 探讨增强造影剂对食管癌调强放疗计划的影响.方法 对10例食管癌进行CT平扫与增强造影,分别设计调强放疗计划,比较两个计划靶区剂量分布和危及器官的剂量体积受量.结果 增强影像计划中靶区最大剂量、最小剂量、平均剂量及中位剂量较平扫影像计划中平均高30cGy,适形度指数和均匀性指数无差异,增强影像计划中双肺平均剂量、心脏平均剂量受量高.两种计划中双肺V20、V30和脊髓最大剂量无差异.结论 造影剂对食管癌调强放疗剂量有影响,但影响较小,可以直接在增强影像上设计放疗计划.
Objective To evaluate the effect of contrast agent on IMRT treatment plan for esophageal carcinoma. Methods Ten patients of thoracic middle segment esophageal carcinoma, treated with IMRT, were planned by CT image with contrast agent or not. Isodose distribution of PTV and dose-volume histogram of organ at risk were compared. Results The maximum dose、the minimus dose、 the mean dose and the median dose of PTV have a mean increase of 30 cGy in plan with contrast agent than in plan without it. No significant difference was found in the maximum dose of spinal cord、the V20 and V30 of lung 、the conformal index and the homogeneity index between the two plan. The mean dose of heart and lung were spared more in plan without contrast agent. Conclusions On the IMRT treatment plan for esophageal carcinoma, contrast agent has little effect and can be neglected.
