Alere’s Personal Health Support approach is not just maximizing “touches” to as many individuals as
possible; it requires a disciplined approach to defining who can benefit from what kind of “touches” and then executing those specific interactions.Our Personal Health Support approach matches intervention resources to available opportunities so that improved health outcomes are achieved in a cost- effective manner.
As part of our engagement,Alere’s care managers work with the participant to develop a care management plan that identifies specific clinical and administrative intervention opportunities that are likely to offer the best prospects for health improvement and cost reduction for each individual.This allows our care managers to prioritize where to start with each participant,specifically direct messages for suggested action to participant specific resource needs (e.g.,pharmacy,a specialty disease management
program,case management,social services).
The primary goal of each diseas management program is to encourage,support and reinforce the participant’s compliance with their physician’s plan of care.The care manager reviews the physician’s plan with the participant early in the program.The care manager determines if the participant has a thorough understanding of the plan of care and addresses any barriers to compliance.The process of reinforcing the physician’s plan of care with the participant takes place during every interaction and is a core element of the Alere disease management program.Continuous re-stratification assures participants are always receiving the most appropriate interventions and yields the best health improvement opportunities.
