He failed to pass the examination through carelessness .他由于粗心,考试失败了（考砸了）.这里的failed是过去式,考试考完了,才会知道失败的结果,所以用过去时.
He was failed to pass the examination through carelessness .这里翻译不通的.was failed是被动语态,不能翻译成我被失败了,所以此处不能用被动语态.
