did +what he could ，这是动宾结构，而且宾语是what引导的宾语从句，本来完整的应该是what he could do，但是因为前面有一个 did 所以就省略了do。
另外，to save it在这里是作目的状语，而不是你理解的can后加do..
你可以把句子断开来多读几遍，理解理解the doctor did what he could ...
