可以
fill in the form是填表,转换成被动式the form should be filled in
而in black是用黑色笔（填写）
两个in在不同的固定短语中,表达不同的意思,不冲突.
但我认为也可以直接说the form should be filled in black.
此处把fill做及物动词,直接加宾语,in black为固定搭配
